North Bay Regional Health Centre (NBRHC) is a unique health care organization with three primary roles: providing acute care services to North Bay and its surrounding communities, functioning as the district referral centre providing specialist services for smaller communities in the area, and it’s the specialized mental health service provider serving all of northeast Ontario. At more than 400 beds, our state-of-the-art regional health centre has over 150 physicians, 2300 employees and 250 volunteers.

What information am I required to submit the week before my first day?
- Checklist - New Hires
- Confidentiality Agreement (Read and sign)
- Respectful Workplace Agreement (Read and sign)
- Fitness Centre Waiver (Read and sign)
- Tax Forms (TD1 click here / TD1ON click here)
- HOOPP (Healthcare of Ontario Pension Plan) Beneficiary forms
- Waiver of Right to Join the Healthcare of Ontario Pension Plan (this form is only required if you do not wish to enroll in HOOPP).
- Parking/Access Request Form
- Preplacement-Assessment-Occupational Health
- Group enrolment form Optional Life (for permanent full time only)
- A copy of certifications, registrations and diploma/degree specific to the posting
- Signed Offer of Employment Letter
- Void Check or Confirmation of Banking Information from an Ontario bank.
- Police Record Check (Please speak to the HR Office to help you distinguish the type of police check required)
- A Copy of your Social Insurance Number Document (Card, letter, other government-issued)
- Current BLS/CPR/First Aid, if required (please refer to job qualifications) | For more info on requirements and community courses offered, click here.
- Driver's License and/or Driver's Abstract, if required
- Proof of previous hours worked, if applicable. Please note that letters from past employers must include the following:
- Job Title Held (RN, RPN, etc.)
- The dates of employment (start and end date)
- Status held (part time, full time, casual)
- The total number of hours worked (from start date to end date)
- Written on company letterhead
- Submitted within 31 days of your date of hire
